8/5/03: After literally HOURS of runaround by CSC employees who sounded like they're on serious drugs I finally got to Donna White, CSC Houston, and her direct fax is 281-878-4882.
She told me because I faxed it to the number for the PofA, my fax was ignored/thrown away, whatever - it was not processed.
I sent the fax again, directly to her.
