There are various formats. Your court might also have pleading paper at their web site for download.
Check your LOCAL rules for specific requirement. In AZ the name of the court can't be above line 6, do have a look at those rules.
You'll also see whether you have to hole punch your complaints, provide a folder, complete the civil cover sheet, ... Often there are exceptions for Pro Ses.
I highly recommend you either download through PACER or order from the court a complaint written by an attorney, possibly for a case similar to yours.
*Normally* you can expect an attorney to submit properly formatted complaints.
